I suffer terribly from airsickness. I am not frightened of flying, but as soon as we have taken off I go green and get an immense feeling of nausea. I can't eat during a flight – whether it's for two hours or ten. The symptoms are aggravated by watching the in-flight film or reading. I have tried travel sickness pills, but they make my mouth dry.

Answer:

Many people suffer from this problem. Although you say you are not frightened of flying, I believe this is probably the root of the problem. Phobia about air travel has increased notably in the aftermath of 11 September, with reports of other air accidents and the constant potential for turbulence adding to the problem.

One of air travellers' main fears is that when accidents happen there are few survivors. When travelling by car, train or boat you have the option of jumping out if there is an emergency. On a plane, however, the only choice is to remain where you are.

This, combined with some people's fear of heights, can make the situation desperate. The classic symptoms of phobia are palpitations, rapid breathing ( hyperventilation), numb or cold hands, tingling in the fingers, cold sweat on the forehead and - the problems you describe - nausea and loss of appetite. Some people suffer from diarrhoea and even involuntary urination. These symptoms are all indicative of fear - a panic attack.

Travel sickness drugs are unlikely to work in this situation. In fact, your dry mouth is another symptom of being afraid. Fear and similar emotions, including grief and rage, cannot be treated with medicines, although certain drugs, such as sedatives and beta blockers, may help control them.

The most effective approach is to deal with fear through psychological therapies, such as neuro linguistic programming (NLP), hypnosis and Thought Field Therapy, meditation and physical exercises such as yoga, aerobics and pilates. All these train the mind to overcome phobias. You can also reduce the effects of this condition by planning ahead and taking measures that will help you to stay calm.

* For 15 days before your journey, take l50mg magnesium, which calms the mind and restores confidence.
* For a week before you travel, go to bed early and take Biorefax or Ashwanite at bedtime to relax and improve the quality of sleep.
* Four to five days before you travel, make sure that you have packed and have organised things at home, so that there is no last-minute rush or panic.
* Avoid foods and drinks that induce tension, including alcohol, coffee, excess salt, fried food, heavy meals, and sugary foods such as chocolates and sweets.
* Drink two litres of still pure water daily.
* Walk in the fresh air for an hour every day.
* Practise retention breathing twice daily for l5 to 20 minutes each time: inhale for a count of three, hold your breath for three and exhale slowly to a count of six.Pause for three, then start the cycle again. Initially, this may cause you to feel a little anxious, but with practice you will find it very effective.

On the day of your journey

* Just before getting on the plane, take ten drops of Valerian Tincture in a cup of water to calm you down.
* When you get to your seat, practise retention breathing. Repeat this during takeoff. Drink sips of water. Try to take a personal stereo with you so that you can play a relaxing tape once the plane has taken off. Or tune in to easy listening or classical music on the in-flight system. Follow the same routine whenever there is turbulence.
* Take lavender essential oil with you and sprinkle a little on a handkerchiefs o that you can inhale the smell whenever you feel nauseous. Or you can sniff a cut fresh lime, which has the same relaxing effect.
* Massage your neck and shoulders using your thumb and fingers. Rub along the sides of your neck particularly. There are details of how to do this on my website. Do this several times during the flight for one to two minutes.

In December 2003, on holiday abroad, I suffered a severe attack of shingles, which covered my neck, head, back, shoulder, arm, face and chest on my right side. My doctor at home told me the shingles had attacked two parts of my nervous system, when it usually only attacks one. I took my several months to recover: my neck was painful and my right arm had a constant painful ache like hitting yourself with a hammer. It was also weak for about five months. Now those symptoms have returned. Is this still the effect of the original illness or is there a possibility of the shingles returning? And can you tell me what I can do to alleviate it please?

Answer:

Shingles are caused by the varicella-zoster or V-Z virus, which is the same as the chickenpox virus: varicella is chickenpox. Shingles, or zoster, appears in people who have previously had chickenpox. The initial infection produces chickenpox, a common and usually mild disease of childhood where watery eruptions on the skin are preceded by sore throat and bronchitis. The virus then gets lodged in the body in ganglia, collections of nerve cells outside the spinal cord, located a long the spinal column the spinal cord. Once there, it goes into a period of hibernation, taking the minimal nutrition it needs from the nerve cells. It can lie dormant for decades '?without showing any symptoms or discomfort whatsoever.

Years later, when immunity to the virus is lost or the immune system is weakened by illness or age, the viral cells can multiply rapidly and spread down a nerve and nerve branches until they reach the skin surface. That isshingles , which causes degenerative changes in the skin similar to chickenpox, and can be extremely painful. The watery blisters become cloudy as white blood cells accumulate and the debris from destroyed skin is deposited. The only good thing is that when the blisters burst and dry, the hard crusts peel off without leaving a mark in the majority of cases.( The reason this virus leaves no long term scaring in most people is because the infection is restricted to the skin surface, unlike smallpox now eradicated- which attacked deeper layers of skin and left indented scars in survivors.) Generally, as your doctor says, only one nerve and its branches are involved, most frequently the one in the middle of the trunk. The nerves in face and neck are less commonly involved.

In your case, the branches of the nerves that feed the shoulder, arm and hands are also involved. I suspect that after your initial infection, the virus must have lodged in multiple ganglia including the neck area. The nerves involved are usually sensory which is why you gel all sorts of sensations such as burning, pain, tingling and itching and the area of the skin feels very sensitive to touch. On rare occasions, motor nerves (those responsible for voluntary movement are involved and sufferers may experience weakness or loss of power in the arm, as you report. Even when the skin eruptions are completely gone, the pain and discomfort may continue for weeks or years, depending on the damage the virus causes as it moves a long the nerve. The more severe the attack, the longer the symptoms persist. Once the pain and burning subsides, however, you can be assured that the attack is over. In your else, the return of the pain in your neck and right arm could be nerve pain caused by problems in that area. Here are some suggestions. In the initial stages of an attack, antiviral drugs are useful which your doctor will prescribe.

At your stage, I would tend to focus on supporting the immune system and treating the neck and upper back.

*Avoid yeast products, coffee, alcohol, citrus fruits, canned products, excess sugar ... Make sure you sleep well to replenish your energy stores: take 1 tablet of Brento at night for one month to relax and get into the habit of good sleep.

* Boost your immune system-with one tablespoonful of Bioprash in a little milk with some Manuka Honey, daily for three months.

* Take one 15 mg tablet of zinc citrate daily for two months'.Take a multivitamin and mineral daily for two month!.

My Itchy Legs Keeps Me Awake At Night?0

For some months, at night, I have had the feeling of something crawling under the skin of my right leg, from the knee to the top of my thigh. It gets so bad that the itchiness turns to pins and needles and invariably keeps me awake. Could you tell me what the problem is and what can be done about it, please?

Answer:

There is a small chance that you could be suffering from a neurological condition, so I do suggest that you tell your doctor about the pins and needles and, if necessary, you will then be referred for the appropriate tests. However, considering your description, there is one condition that comes to my mind straight away. To explain, I must give you the history of a gentleman who had similar problems. This was a French business man who had been advised to have surgery on his back because he had a slight protrusion of a lower lumbar disc. His doctors in France had pinpointed this as the cause of the symptoms that disturbed his sleep and prevented him playing golf, his favourite pastime. The symptoms were exactly the same as yours - the feeling of insects crawling under his skin plus pins and needles, located in his thigh, from the hip to the top of the knee. According to his doctors, these symptoms were signs of sciatica. Since he had been in discomfort, for months and physiotherapy produced no good result, surgery was the only option.

The date for his operation was fixed when his boss (a patient of mine) asked him to come to see me for a second opinion. Although he was reluctant to cancel the surgery and consider another diagnosis, I was able to convince him that the diagnosis was wrong and that surgery would not solve his problem. Even if the bulge on that particular lumbar disc (between L4 and L5) scratched the nerve root, the sensation or pain would not be felt above the knee; it would go down the side of the calf to the ankle. Where he experienced the sensation in the thigh comes from upper lumbar vertebrae (Ll, L2 and L3). Operating on a lower lumbar disc simply would not affect the pain.

I asked him how much water. coffee and alcohol he drank and how much salt he consumed. He drank hardly any water but several cups of coffee daily and wine regularly; he also liked salt. I explained that these dietary habits could lead to dehydration and soreness in the kidney (upper lumbar) area. The kidneys are actually embedded in the lumbar muscles, so whenever there is a problem with them, the lumbar muscles get sore and very tight. This can irritate the nerve roots in that area and cause this type of symptom. Sometimes it even results in lumbago or lower backache, as lumbar muscles are attached to the lower part of the spine - the sacrum. His skepticism evaporated as he realised the logic of what I was saying.

When I examined his back and touched the lumbar area, he felt the pain immediately. I massaged the whole of the spine, focusing on the lumbar region. When he got up from the couch, the sensations in his thigh were gone. He touched the areas that had bothered him and they were no longer sensitive. He came to my clinic weekly for about a month for massage and yoga. Almost three years on, the symptoms have never returned. The message is that one must always investigate such symptoms and, if possible, give nature a chance to heal the body before rushing to drastic measures. I have detailed similar cases involving unusual back and neurological problems in my Ultimate Back Book.

Here are my recommendations

How Can I Reduce My Unwanted Hair?0

Question:

I'm l9 and ever since I hit puberty I've had a lot of body and facial hair- I can mask it by bleaching but over the past few years the hair has increased on my neck, stomach and lower back. Although the body hairs are quite fine, I am very self-conscious about the thicker hairs on my face' I wonder why this is happening and whether there are any changes I can make to reduce the hair.

Answer:

There are two types of excess hair growth in women. The first is a generalised increase all over the body, which may be what you have. The other type - hirsutism - is distributed in a male pattern, so females develop a beard and chest hair.

Normally a woman has thick hair on the scalp, pubic region and underarms, and fine hair in moderation all over the body. Trouble starts if the adrenal glands become overactive, due to stress or overstimulation of the pituitary gland (the hormonal headquarters in the brain). The adrenals release male hormones (androgens) that can be converted into testosterone in various cells of the body, which need not be the male testes. So women, who have a little testosterone anyway, may develop more under stressful conditions, or if they have pituitary problems.

The adrenals also release stress hormones. Interestingly, these are mostly male in nature - like testosterone. Evolution designed men for 'fight or flight'. Now that women are suffering the same pressures as men, androgens may be released from the adrenals at the same time as stress hormone gland convert to testosterone a (Vigorous exercise also stimulates the adrenals to release androgens, which can then convert to a form of oestrogen; a useful tip for menopausal women.)

Hirsutism is also a common symptom of polycystic ovary syndrome (PCOS). In this case, androgens are Produced by the ovaries and also in fat cells. In PCOS sufferers, these male hormones suppress menstruation and cause hair growth.

You must consult your doctor if you think your hormones are unbalanced. Signs of this include irregular periods, acne, rapid weight gain, irritability and mood swings not connected to PMS. You will probably be given blood tests to find out if there are

signs of excess androgen production, although I should warn you that these are not always very accurate in the early stages. If hair growth is accompanied by some or all of these symptoms, it suggests either PCOS or release of androgens from the adrenal glands. Your doctor may prescribe a type of contraceptive pill if your periods are irregular; otherwise testosterone-suppressing drugs may be considered.

Because stress is likely to be the cause of or a factor in your problem, i suggest the following

* Avoid coffee and alcohol, excess salt and sugar (which are commonly found in prepared foods of all kinds); these ingredients can add to your existing level of stress.

* Avoid late nights and gen as much sleep as possible.

* Walk regularly in the fresh air of the countryside' or in parks if you live in the city.

* Avoid yeast products (such as bread, Marmite, Pasta, pizza, cakes), as these an make you tired.

* Eat rhubarb, Pomegranate and yams, which contain plant estrogens.

* Take one capsule of aloe vera and one tablet of shatavari twice daily for three months, to help to balance hormone levels.

* Rub lard right into the skin under the worst hair growth. Do this twice a week for three months. Lard destroys the hair roots and facilitates hair loss.

* Practise yoga to help you relax Find a good class and also do some at home-in the evenings, always finishing with the corpse pose. You will find instructions in Therapeutic Yoga, the book I co-wrote with Jiwan Brar.

* Listen to a relaxation, tape in bed (there are good ones available in complementary health stores or you can buy my own tape, Dr Ali's Yoga and Meditation programme, Sanogenetic Solutions).

The hamstrings are a muscle group which are situated at the back of the thighs and are commonly involved in injury from vigorous activities and sports. Hamstring injuries are more commonly involved in areas of the muscle which are closer to the buttock and on the outside of the back of the thigh. There are three muscles in the hamstring group and they do not have common names, being termed semimembranosus, semitendinosus and biceps femoris. The latter is the muscle most commonly involved in typical sporting injuries.

Injuries to the hamstring muscle group is classified by using grades of severity: grade one being a mild muscle strain with a number of torn muscle fibres, grade two is rated as a moderate strain with a noticeable loss of muscle strength and grade three is a complete tear of a muscle. The most common position for injuries to occur is where the muscle and tendon join on the upper part of the muscles near the buttock, although in the biceps femoris the muscle-tendon junction occurs over a significant length. The tendons themselves do not usually get injured within their own structure unless there is some already existing pathology.

The origin of the hamstring tendons is the bones in the buttock which we sit on, known formally as the ischial tuberosities. The tendons are attached to this bony area and a violent movement into hip bend with a large overall movement range can avulse (tear off) the tendon's junction from its bony bed. Water skiing is a risky activity for this problem. Most hamstring injuries occur in younger people who overwhelmingly perform athletic activities, with typical sports for this injury being field sports, contact sports, track activities such as sprinting, football or rugby.

The hamstrings start at the ischial tuberosity in the buttock, run down the back of the thigh and insert into varying places on the tibia or fibula. When these muscles are contracting and lengthening at the same time (eccentric contraction) such as in track events and rugby, there can be a high risk of injury. Direct muscle blows can result in contusions to the tissues while water skiers are more prone to avulsion injuries as they fall because they undergo rapid hip flexion with their knees straight. Onset of a hamstring strain is typically sudden and often when the person is moving quickly, with an audible pop in the muscle often reported


Immediate pain is noticed in the posterior thigh area and people may be vulnerable early in the process before they are warmed up or later on when they may be becoming tired. If the damage is not severe then function can be preserved although with pain such as on stair climbing or walking up a hill. Examination of the back of the thigh may reveal little but it may well be painful if the patient is asked to bend their knee and the physiotherapist resists the movement. Rupture of one of the hamstrings may show by it balling up on contraction along with reduced strength on assessment.

The likelihood of suffering from an injury to the hamstring is thought to be related to factors such as tiredness, inadequate warm up, flexibility restrictions or if the strength ratio between the hamstrings and the quadriceps is incorrect. Having previously incurred a hamstring injury is enough to raise the risk of having another one. How an injury will be treated depends entirely on its severity with one end of the spectrum seeing the physio progress the patient speedily on to strength work and at the other end of the spectrum some injuries require the intervention of a surgeon.
